Targeted Mental Health Case Management is a service provided to qualifying Medicaid recipients to assist them in accessing resources that can considerably improve the quality of their lives. Our Case Managers help clients gain access to services needed to meet their mental health needs, achieve recovery, and live independently in the community Examples of services provided include: assisting individuals and families connect to medical, psychiatric, housing, and financial services. Case Managers advocate for clients in different settings, including school, legal and vocational systems.
